Notes:
- Value Added Tax has been applied on the above amount.
- Maximum revisions for all items above is 2x (two times). Additional revisions will be charged proportionally.
- Cancellation after project started (man-hours spent) will be charged at minimum 20% from total amount.
- Cancellation after signing will be charged at minimum 40% from total amount.
- Project will effectively commence after quotation has been signed and approved by counterparty.
- Estimate must be approved before commencement of job and any change in specifications will require a complete revision.
- This is an estimate and it may include items, the cost of which is difficult to predict exactly. It is our best appraisal of the cost of the work on the date submitted, and is subject to reasonable increase or decrease.
- We are not to be held responsible for the delays caused by strikes, fire or other causes beyond our control.
Finance Term:
- Maintenance / Retainer items will be invoiced on a monthly basis.
- One-time production items will be invoiced partially upon item completion.
- Terms of payment is within 30 days after invoice submission. Any payment delay will be charged 2% per month.

Jika saya terbukti memalsukan pernyataan tersebut setelah saya mengikuti proses casting dan menandatangani Talent Release sebelum shooting berlangsung, maka saya siap untuk bertanggung jawab atas kerugian yang telah dikeluarkan oleh pihak CV PERSUASENI KREASINDO dengan mengganti biaya produksi minimum sebesar Rp. 100.000.000,- (Seratus Juta Rupiah), maksimum sebesar total seluruh biaya produksi ditambah dengan biaya lainnya (jika ada) yg timbul dan dibebankan oleh klien CV. Persuaseni Kreasindo kepada CV PERSUASENI KREASINDO atas kesalahan saya tersebut.
Apabila saya terbukti memalsukan pernyataan setelah proses shooting, maka saya bersedia untuk mengganti seluruh kerugian, minimum sebesar total seluruh biaya produksi dan maksimum adalah sebesar seluruh biaya produksi ditambah dengan biaya kerugian lainnya yang akan ditimbulkan seperti, biaya pemasangan iklan di media yang hangus karena permasalahan ini, biaya legal, dsb. Adapun keseluruhan biaya tersebut berkisar antara Rp. 500.000.000 (Lima Ratus Juta Rupiah) s/d Rp. 5.000.000.000 (Lima Milyar Rupiah).
Saya setuju bahwa CV PERSUASENI KREASINDO yang memproduksi iklan tersebut di atas, memiliki hak penuh atas seluruh hasil produksi baik film, video dan atau foto maupun bentuk media lainnya termasuk untuk menggunakan kembali hasil produksi tersebut.
CV PERSUASENI KREASINDO dan klien CV PERSUASENI KREASINDO juga memiliki hak siar dan hak intelektual penuh untuk menggunakan hasil produksi tersebut dalam waktu dan tempat yang tidak terbatas.
Saya, sebagai model menjamin bahwa : SELALU SIAP DAN SELALU TEPAT WAKTU dan akan menyediakan waktu kapan saja dibutuhkan untuk kelancaran produksi iklan ini, seperti : casting ulang (re-cast), workshop (latihan), pengepasan baju (wardrobe fitting), merubah gaya rambut (hair style), pengambilan gambar (shooting) hingga selesai dan lain-lain.
Saya memahami bahwa saya akan, dengan kemampuan terbaik saya, mematuhi jadwal yang telah disepakati. Tambahan, saya setuju, dengan kemampuan terbaik saya, untuk membuat diri saya selalu siap jika diperlukan, untuk merekam ulang suara saya dan/atau merekam sulih suara dan melakukan pekerjaan suara yang diperlukan setelah akhir pembuatan video. Jika saya tidak dapat melakukan pekerjaan suara seperti itu, saya memahami bahwa CV PERSUASENI KREASINDO dapat membuat perjanjian dengan orang lain untuk merekam ulang dialog saya dan/atau merekam sulih suara dan menggunakan pekerjaan suara ini di atas gambar saya atau dengan cara apapun yang dianggap pantas oleh CV PERSUASENI KREASINDO.
Saya dengan ini memberikan izin dan hak kepada CV PERSUASENI KREASINDO untuk memotret saya dan merekam suara saya, penampilan, pose, aksi, dan lakon saya, serta menggunakan gambar, foto, siluet, dan reproduksi lain dari rupa fisik saya sehubungan dengan video / foto terkait Project ini.
Dengan ini saya berikan kepada CV PERSUASENI KREASINDO dan klien CV PERSUASENI KREASINDO hak abadi untuk menggunakan semua foto, film, video serta rekaman dan rekaman soundtrack yang mungkin dibuat tentang saya atau suara saya, dan hak untuk menggunakan nama atau rupa saya dalam atau sehubungan dengan pameran, iklan, pemanfaatan dan/atau publikasi video dan foto. Saya selanjutnya memberikan hak untuk mereproduksi dengan cara apapun rekaman apapun termasuk semua instrumen, musik, atau efek suara lainnya yang saya hasilkan, sehubungan dengan produksi dan/atau pascaproduksi video dan foto.
Saya setuju bahwa saya tidak akan mengajukan klaim, tindakan, gugatan atau tuntutan dalam bentuk apapun, termasuk namun tidak terbatas pada yang didasarkan pada pelanggaran privasi, hak publisitas atau hak-hak sipil lainnya, atau untuk alasan apapun sehubungan dengan penggunaan CV PERSUASENI KREASINDO dan klien CV PERSUASENI KREASINDO yang sah atas kemiripan fisik dan suara saya dalam video / foto yang diproduksi / yang dihasilkan oleh CV PERSUASENI KREASINDO.
